Job summary

Tennessee Quality Care Hospice is seeking a PRN RN to join our team in Martin, TN. We offer flexible hours, a supportive environment, and the opportunity to make a meaningful difference in patient and family lives.

Responsibilities include performing nursing assessments, coordinating care with the interdisciplinary team, delivering direct patient visits, and maintaining accurate charts. Valid RN license, CPR, and clean driving record required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Maintains a current CPR certification.
• Must possess the ability to deal tactfully with patients, family members, visitors and staff.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Assist with patient and family caseload throughout nursing process phases: assess, determine eligibility, admission, planning, implementation and evaluation.
• Obtain data from patient and families across physical, psychological, social and spiritual factors and coordinate interventions.
• Communicate with attending physician, interdisciplinary team and other agencies to coordinate optimal care.
• Provide visits to reassess patient needs and update the care plan.
• Perform nursing procedures as directed by the physician and provide patient care.
• Perform clinical pronouncement of death and support for family.
• Maintain accurate patient records, progress notes, reports and care plans.

What Youll Do

• Assist with the responsibility for patient and family caseload including the assessment determine eligibility admission processing planning implementation and evaluation phases of the nursing process


• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact patient and familys needs and initiate intervention by other members of the interdisciplinary team


• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care


• Provide visits to re assess the patients needs and update the plan of care


• Perform therapeutic preventative and rehabilitative nursing procedures as directed by the attending physician


• Perform clinical pronouncement of death and provide support to family


• Maintain accurate patient records charts progress notes reports and prepare care plans timely

Qualifications
