Job summary

Addus HomeCare in Martin, Tennessee is seeking a licensed PRN Social Worker to join the hospice team and provide comprehensive psychosocial care to patients and families in their homes. The role collaborates with clinicians to coordinate resources and services across the care plan.

TN-licensed MSW with LMSW/LCSW is required, with at least one year of health care or hospice experience. Shifts are as needed, primarily Monday–Friday, serving Martin and surrounding counties.

Qualifications

• Master of Social Work (MSW) required and state licensure in Social Work (LMSW or LCSW) in Tennessee.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance are required.

Responsibilities

• Performs psychosocial assessment of patients and families/caregivers in the hospice program and ensures identified needs are addressed in the plan of care.
• Provides counseling to patients and families to support coping and stress management.
• Assists with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver; connects patients/families with community resources.

Tennessee Quality Care Hospice is seeking a licensed PRN Social Worker to join their growing team in Martin! This role provides counseling, resources, and coordination with our clinical team to ensure holistic care for both patients and families in the comfort of their homes.

MSW and TN state license required.

Schedule: As Needed. Shifts will be Monday-Friday 8:00am-4:30pm shifts.

Territory: Martin, TNand surrounding counties.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies.
