Job summary

The Tennessee Quality Care Hospice is seeking a licensed PRN Social Worker to join our growing team in Martin. This role provides counseling resources and coordinates with our clinical team to ensure holistic care for patients and families in their homes.

A MSW with Tennessee state licensure is required. Responsibilities include psychosocial assessments, coordination with care plans, assisting with access to community resources, and supporting patients and families throughout the hospice

Qualifications

• MSW with Tennessee licensure required (LMSW or LCSW).
• Minimum 1 year experience in health care or hospice.
• Valid driver’s license and proof of insurance.

Responsibilities

• Performs psychosocial assessments of patients, families, and caregivers and integrates needs into the plan of care.
• Assists with residence-based assessments and alternative plans when safety is a concern.
• Supports team members, ensures service delivery quality, and documents patient services accurately.
• Facilitates applications for SSI, Hospice Medicaid, and waivers; connects with community resources.

Qualifications

Social Work DegreeTennessee Masters Degree in Social Work requiredCurrent State Licensure in Social WorkTennessee LMSW or LCSW requiredMinimum of one years experience in health care or hospice settingValid drivers license and proof of insurance is required
