Job summary

Tennessee Quality Care Hospice in Columbia, TN is seeking a licensed PRN Social Worker to join our team delivering compassionate, home-based psychosocial care. You will assess needs, provide counseling, and coordinate with the clinical team to support patients and families in their homes.

Requirements include Tennessee Master’s in Social Work, LMSW or LCSW licensure, and at least one year in health care or hospice. A valid driver’s license is required.

Qualifications

• MSW degree is required.
• Tennessee LMSW or LCSW licensure required.
• Minimum of one year of health care or hospice experience.
• Valid driver’s license and proof of insurance.

Responsibilities

• Conduct psychosocial assessments for patients and families in their home or residence.
• Provide counseling and support to patients, families, and caregivers.
• Assist with applications for benefits such as SSI, Medicaid, and waivers.
• Document services accurately to reflect the plan of care.
• Coordinate with the clinical team to address identified psychosocial needs.
• Serve as liaison between patients, families, and community resources.

Schedule: PRN. Shifts will likely be Monday-Friday 8:30am-4:30pm

Territory: Columbiaand surrounding counties.

Qualifications:

• Social Work Degree
  • Tennessee: Master’s Degree in Social Work required.
• Current State Licensure in Social Work

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies.
