Job summary

Addus HomeCare in Columbia, TN is seeking a licensed PRN Social Worker to join our home-based hospice team. You will perform psychosocial assessments, provide counseling and coordinate with a clinical team to support patients and families in their homes.

Required are an MSW and TN licensure, plus at least one year of healthcare or hospice experience. A valid driver’s license and insurance are required; shifts are PRN, typically weekdays during daytime hours in Columbia and surrounding counties.

Qualifications

• MSW and TN state license required.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicare, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies.
