Job summary

Fusion Healthcare Rehabilitation and Nursing Center in South Hadley, MA is seeking a social worker to support residents’ physical, cognitive, and psychosocial needs. The role involves developing care plans, coordinating admissions and discharges, and collaborating with interdisciplinary teams to ensure comprehensive resident care.

The ideal candidate holds a BSW/MSW or related degree and has at least one year of social work experience, preferably in long-term care or sub-acute settings.

Qualifications

• BSW or MSW preferred or a bachelor’s degree in a related field.
• At least one year of experience in the social work field is required.
• Experience in long-term care and/or sub-acute settings preferred.

Responsibilities

• Maintain or improve residents’ ability to manage daily physical needs and psychosocial needs.
• Identify individual social and emotional needs of residents.
• Develop and maintain individualized care plans.
• Participate in admission, re-entry, and transfer planning.
• Join interdisciplinary care plan meetings to address social and emotional needs.
• Collaborate with the finance team to ensure proper documentation for residents.
• Assist with discharge planning and arrange DME/home care for safe community return.
• Apply knowledge of age-specific developmental factors in planning social services.
• Plan services for residents with therapeutic, neurological, dementia, dialysis, IV therapy, or infectious disease needs.
• Follow up on admission inquiries and assess prospective residents for facility placement.

Duties and Responsibilities:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

• Identifying individual social and emotional needs

• Developing and maintaining individualized care plans

• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay

• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.

• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.

• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
