Job summary

Skilled Nursing of New Jersey in Elizabeth, NJ, is seeking a full-time LSW/CSW Social Worker to join our care team in a sub-acute and long-term facility. You will conduct admissions and ongoing assessments, support residents through adjustment, and participate in care planning and MDS 3.0 processes.

The ideal candidate holds a NJ CSW/LSW license, is highly organized, and demonstrates strong writing and communication skills.

Qualifications

• Must hold a NJ CSW/LSW license in good standing.
• Must be able to multi-task and stay organized.
• Must have a passion for the senior population and strong writing skills.
• Must be an effective communicator and a team player.
• Familiarity with Point Click Care (PCC) is a plus but not required.

Responsibilities

Job description

Full-Time LSW/CSW Social Worker for a sub-acute and long-term care facility located in Elizabeth, NJ to join our team of professionals.

Duties include, but are not limited to:

• Admission and Ongoing Assessments/Documentation
• Helping with Adjustment
• MDS 3.0
• Care Planning
• Assisting with Medicaid Applications and other Financial Matters
• Advocacy
• Crisis Intervention
• Discharge Planning

To qualify for this position, the ideal Candidate:

• Must have a NJ Social Work Certification CSW/LSW License in good standing
• Must be able to multi-task and organize
• Must have a passion for the senior population
• Must have strong writing skills
• Must be an effective communicator
• Must be a team player
• Familiarity with Point Click Care (PCC) is a plus but not a requirement.

We offer a competitive salary 60,000 depending on experience and a generous benefits package including health, dental, matched 401(k) and PTO Days.
