Job summary

Andwell Health Partners seeks a Medical Social Worker to support Hospice/Home Health in Bangor, ME. This role involves delivering social work services in inpatient and community settings with per-diem scheduling.

Responsibilities include assessing patient needs, developing and implementing care plans, and serving as an educational resource for staff, while supporting Hospice and Home Health services across Bangor and surrounding towns.

Qualifications

• Degree from a CSWE-accredited school of social work.
• Maine Board of Social Work Licensure: at least LSX.

What You’ll Do:

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
• Support Hospice, and Home Health

What You’ll Bring:

• Degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure: at least LSX
• Two years of Medical Social Work preferred
• Current CPR card required.
• A valid Drivers License and reliable car with insurance.
