Job summary

Andwell Health Partners in Presque Isle, ME is seeking a Medical Social Worker for home healthcare services in the community and patient homes. This 32-hour position focuses on assessing and planning care with families and the health team.

You will develop and implement a care plan, support staff education, and coordinate Hospice and Home Health services with emphasis on autonomy and meaningful patient interactions in home settings.

Qualifications

• Master's in Social Work from a CSWE-accredited program.
• Maine Board of Social Work Licensure: at least MSW.
• Two years of Medical Social Work preferred.
• Current CPR certification required.
• Valid driver's license and reliable car with insurance.

Responsibilities

• Assesses and identifies the client’s holistic health care needs in family and community contexts.
• Evaluates social/emotional factors related to illness, care needs, and treatment response.
• Develops a care plan with physician orders and measurable outcomes with client/family involvement.
• Implements patient care activities toward independence in living situations.
• Serves as a resource for staff education and participates in case conferences as needed.
• Supports Hospice and Home Health services.

What You’ll Do

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
• Support Hospice, and Home Health

What You’ll Bring
