Andwell Health Partners seeks a Medical Social Worker (MSW/LMSW/LCSW) to join our Hospice and Palliative Care team in Lewiston, ME. This full-time role provides community-based support in homes and facilities, under physician orders, to improve patient and family outcomes.

You will assess needs, develop care plans, and collaborate with health care providers to support independent living and quality of life for patients receiving hospice or home health services.

Qualifications

• Master’s degree from a CSWE-accredited school of social work.
• Maine licensure as MSW or LCSW is required.
• Current CPR certification is required.
• At least 1 year of MSW experience in a health care setting.
• Valid driver's license with reliable, insured vehicle.

Responsabilités

• Assesses client holistic health care needs within family and community.
• Evaluates social/emotional factors related to illness and care.
• Develops care plans under physician orders with measurable outcomes.
• Implements care activities toward independent living for patient/family.
• Serves as resource for staff education and training; participates in case conferences.
• MSW/LCSW supports hospice, palliative care and home health.

What You’ll Do:

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
• MSW/LCSW to support Palliative Care, Hospice, and Home Health

What You’ll Bring:

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure- MSW or LCSW required
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.
