Job summary

Andwell Health Partners seeks a Medical Social Worker to support Hospice/Home Health in Bangor, ME. This role involves delivering social work services in inpatient and community settings with per-diem scheduling.

Responsibilities include assessing patient needs, developing and implementing care plans, and serving as an educational resource for staff, while supporting Hospice and Home Health services across Bangor and surrounding towns.

Qualifications

• Degree from a CSWE-accredited school of social work.
• Maine Board of Social Work Licensure: at least LSX.

Responsibilities

• Assess and identify the client’s holistic health care needs as part of family and community.
• Assess social/emotional factors related to illness, need for care, response to treatment, and adjustment to care.
