Andwell Health Partners is seeking a Medical Social Worker (MSW) for hospice/home health in the Windham/Gorham/Standish, Maine area. The role focuses on assessing holistic health needs, collaborating with families and the health care team, and implementing care plans to support independent living at home.

The ideal candidate holds an MSW or LCSW, Maine licensure, and CPR certification, with at least one year of health care social work experience and a valid driver's license with reliable

• Master’s degree in social work (MSW) from an accredited school.
• Maine Board of Social Work Licensure required: MSW or LCSW.
• Current CPR certification is required.
• Valid driver's license and reliable car with insurance.

Responsabilidades

• Assess and identify client holistically within family and community.
• Evaluate social/emotional factors related to illness and care needs.
• Develop and implement a care plan with physician orders and care team.
• Provide staff education and participate in case conferences as needed.
• Support patients and families toward independent living and recovery.
