Job summary

Andwell Health Partners is seeking a Medical Social Worker to assess holistic health care needs within Norway, Maine's community care settings. You will collaborate with families and the health care team to develop and implement a plan of care.

The role requires an MSW, Maine licensure LSX, CPR, and at least one year of health care social work experience. A valid driver's license and reliable car are also required.

Qualifications

• Degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure- LSX Minimum.
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• Valid driver's license with reliable car and insurance.

Responsibilities

• Assesses and identifies holistic health care needs for the client and family.
• Assesses social/emotional factors related to illness, care, and adjustment.
• Develops care plans under physician orders with measurable outcomes.
