Job summary

Andwell Health Partners is seeking aMedical Social Worker in the Lewiston, Maine area to assess and address holistic health care needs within home health and hospice settings. You will collaborate with families and healthcare teams to develop and implement care plans aimed at improving patient independence and comfort.

The role requires MSW/ LCSW licensure, a current CPR card, and prior social work experience in healthcare.

Qualifications

• MSW degree required and licensure to practice in Maine (MSW/LCSW)
• Current CPR card required
• One or more years of experience as a master’s level social worker in a health care setting
• Valid driver's license and reliable vehicle with insurance

Responsibilities

• Assesses and identifies the client’s holistic health care needs and concerns as part of a family and community.
• Assesses social/emotional factors related to illness, care needs, response to treatment, and adjustment to care.
• Develops a care plan under physician orders with measurable outcomes in collaboration with client/family and health care team.
• Implements patient care plan activities toward independent living for patient/family.
• Serves as resource for staff education and training; participates in case conferences as needed.
