Job summary

Andwell Health Partners in Bridgton, ME seeks a Medical Social Worker to support patients and families facing complex medical conditions. You will provide psychosocial support, counseling, care coordination, and connect them with community resources.

This full-time exempt role requires a degree in social work, licensure eligibility, and strong communication and coordination skills. You will work with physicians, nurses, and an interdisciplinary team to deliver compassionate, quality care.

Qualifications

• Degree in Social Work (or related field) from an accredited program.
• Current social work licensure or eligibility for licensure, as required.
• Experience in healthcare, home health, hospice, or community-based services preferred.

Responsibilities

• Assess patients’ social, emotional, and environmental needs.
• Develop and implement individualized care plans.
• Provide counseling, crisis intervention, and patient advocacy.
