It is designed to engage patients who have extensive barriers to HIV care using incentives. Physically located at Harborview Medical Center in Seattle, the clinic serves over 200 patients, who access primary care on a walk-in basis. Max Clinic serves a patient population that is highly diverse, serving patients from various racial, ethnic, immigrant and LGBTQ communities.PRIMARY JOB RESPONSIBILITIES:• Provide HIV/AIDS intake and case management to adult patients served in Madison Clinic.• Conduct comprehensive assessments of psychosocial, financial, medical support and environmental needs• Develop individualized case management service plans to assist the hospitalized and ambulatory care patient to obtain needed medical, social, financial, legal and home care services, housing, psychiatric and drug treatment and emotional supportREQUIRED POSITION QUALIFICATIONS:A Master of Social Work Degree from a program accredited by the Council on Social Work EducationANDOne year of full-time social work experience in a health care setting or equivalent.
