Job DescriptionThe UW Medical Center-Northwest Social Work Department has an outstanding opportunity for a Senior Social Worker to join us in our new Center for Behavioral Health and Learning building.WORK SCHEDULEFull-Time / 40 hours per weekDay ShiftPOSITION HIGHLIGHTSPartner with patients, their families and the health care team to address medically-related social and emotional needs and provide support and advocacy as these impact the patient's medical condition, treatment, recovery and safe transition from one environment to anotherBe responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention and resource referral(s)Partner with the patient/family and the health care team to coordinate care across the continuum, improving patient care through effective utilization of resources by transitioning from the acute care episode to the post-acute settingABOUT THE CENTER FOR BEHAVIORAL HEALTH AND LEARNINGThe new Center for Behavioral Health and Learning , opening on the UW Medical Center – Northwest campus in 2024, offers an unparalleled opportunity to transform mental health care and to improve mental health care training in our state. Located in North Seattle, the new facility will feature 75 longer-term adult psychiatry beds, 25 geriatric psychiatry beds, 50 medical/surgical beds focused on patients with psychiatric comorbidities, and a state-of-the-art neuromodulation program. It will also house an interdisciplinary training and workforce development program focused on preparing and supporting the next generation of behavioral health providers for Washington State. The six-story building will be a beautiful place to work with lots of windows, natural lighting and open-concept spaces and will offer amenities including a new kitchen and dining area for patients, staff and visitors.Learn more about the Center for Behavioral Health and Learning here.PRIMARY JOB RESPONSIBILITIESProvide specialty consultation to other health care staff and health care teams regarding psychosocially focused aspects of client/patient careProvide psychosocial support to patients and familiesComplete clinical assessments as per department standards for area of serviceIn collaboration with the multidisciplinary team, develop and implement safe and timely discharge plansProvide care coordination for patients with complicated social and medical historiesProvide case management and counseling services in specialized programsMake timely referrals to hospital and/community agencies, including those required by mandatory reporting laws such as child and vulnerable adult abuseREQUIRED POSITION QUALIFICATIONSA Master of Social Work Degree from a program accredited by the Council on Social Work Education and one year of full-time social work experience in a health care setting or equivalent.
