Job summary

Qualifications

• Ability to work within multidisciplinary teams to coordinate patient care.

Responsibilities

• Screens charts and conducts psychosocial evaluations with patients or surrogates.
• Provides individual and group counseling in clinical or community settings.
• Develops treatment plans and coordinates with other disciplines for patient care.
• Assists in discharge planning and post-discharge follow-up as needed.
• Maintains documentation and reports to ensure compliance with policies and regulations.
