Qualifications

• Bachelor's or master's degree in social work from an accredited university.
• Minimum 2 years of experience in case management and social advocacy.
• Bilingual English/Spanish strongly preferred.
• Managed Care experience preferred.

Responsibilities

• Coordinate services for AltaMed Managed Care participants and advocate for participants.
• Maintain daily notes and conduct family conferences.
• Assist with community referrals and maintain regular contact with participants.
• Report incidents as required and ensure accurate documentation.
