AltaMed Health Services Corp. in Montebello, CA, seeks a dedicated case manager to support Managed Care participants, collaborating with providers and nurses to coordinate services and advocate for participants.

Responsibilities include documenting daily notes, leading family conferences, assisting with referrals, maintaining regular participant contacts, and reporting incidents to ensure quality care. A Bachelor’s or Master’s in social work and at least two years of experience are required;

Qualifications

• Bachelor’s or Master’s degree in social work from an accredited university.
• Minimum of 2 years of experience in case management and social advocacy.
• Experience working with the homeless population preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Job Overview

Responsible for AltaMed Managed Care participants assigned to the team. This position works closely with providers and nurses to coordinate services and advocate for participants. Responsibilities include, but are not limited to: daily notes, conducting family conferences, assisting with community referrals, regular contacts with participants, and incident reporting.

Minimum Requirements

• A bachelor’ or master’s degree in social work from an accredited university is required.

• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Compensation

$26.78 - $32.48 hourly
