Job summary

AltaMed Health Services in Montebello, CA 90640 seeks a Social Worker I to coordinate care for managed care participants. You will work with providers and nurses to advocate for clients, conduct family conferences, and manage referrals.

Strong preference for bilingual English/Spanish and experience with the homeless population. The role offers hourly compensation and a comprehensive benefits package, with potential discretionary bonuses.

Qualifications

• A bachelor’s or master’s degree in social work from an accredited university is required.
• Minimum of 2 years of experience in case management and social advocacy required.
• Experience working with the homeless population is preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Coordinate services for AltaMed Managed Care participants in collaboration with providers and nurses.
• Perform daily notes and family conferences.
• Assist with community referrals and maintain regular contact with participants.
• Report incidents as required.

Education

Bachelor’s or Master’s degree in social work

2+ years case management experience

Bilingual English/Spanish strongly preferred
