Job summary

AltaMed Health Services Corp. seeks a dedicated case manager to coordinate services for Managed Care participants.

You will collaborate with providers and nurses to advocate for patients and ensure seamless, effective care delivery. Requirements include a bachelor’s or master’s degree in social work, at least 2 years’ experience in case management and social advocacy, and bilingual English/Spanish is strongly preferred.

Qualifications

• Bachelor’s or Master’s degree in social work from an accredited university.
• Minimum of 2 years of experience in case management and social advocacy.
• Bilingual English/Spanish strongly preferred.
• Experience with the homeless population and managed care is preferred.

Responsibilities

• Daily notes, conducting family conferences, assisting with community referrals, regular contacts with participants, and incident reporting.
