AltaMed Health Services Corp. in Montebello, CA is seeking a Social Worker I to coordinate care for Managed Care participants and advocate for clients.

Qualifications

Responsibilities

• Responsible for AltaMed Managed Care participants assigned to the team.
• Works closely with providers and nurses to coordinate services and advocate for participants.
• Duties include daily notes, conducting family conferences, assisting with community referrals, regular contacts with participants, and incident reporting.
