AltaMed is seeking a Social Worker I based in Montebello, CA to coordinate services for Managed Care participants. You will work closely with providers and nurses to advocate for clients, document daily notes, and facilitate referrals. Strong bilingual English/Spanish is highly valued.

The role emphasizes collaboration within a team, community outreach, and ongoing participant engagement to ensure comprehensive care and timely incident reporting.

Qualifications

• Bachelor's or master's degree in social work from an accredited university.
• Minimum of 2 years of experience in case management and social advocacy.
• Experience working with the homeless population is preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Coordinate services for AltaMed Managed Care participants with providers and nurses.
• Advocate for participants and maintain daily notes.
• Conduct family conferences and assist with community referrals.
• Maintain regular contact with participants and report incidents.
