Job summary

Lakes Region VNA and Hospice seeks a compassionate Social Worker to support Hospice and Home Care patients and families. The role involves assessing psychosocial needs, developing care plans, and providing emotional support to help navigate the complexities of healthcare.

The ideal candidate will hold a master’s degree in social work and have at least one year of healthcare experience, with a commitment to patient-centered care and teamwork in a community-focused organization.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Minimum of one year of healthcare experience, preferably hospice or home care.
• Current NH Social Work license.

Responsibilities

• Conduct psychosocial assessments of patients’ needs, strengths and challenges.
• Collaborate with interdisciplinary team to create individualized care plans addressing social, emotional and financial needs.
• Provide counseling and support to patients and families facing illness, loss and end-of-life issues.
• Assist patients and families in accessing community resources and referrals to appropriate services.
• Respond to crisis situations with appropriate intervention and safety planning.
• Maintain accurate documentation of interactions and care plans in medical records.
