Job summary

Visiting Nurse Home Care & Hospice in New Hampshire is seeking a part-time Medical Social Worker to support hospice and home care patients and families. The MSW will assess psychosocial needs, plan care, and coordinate with the care team.

Candidates hold an MSW and are licensed or eligible for licensure in Maine, with strong communication, CPR, and a valid driver's license. Prior home-based care experience is preferred.

Qualifications

• MSW degree from an accredited school of social work.
• Licensed social worker or eligible for licensure in Maine.
• Minimum of one year of health care experience.
• Home-based care experience preferred.
• Demonstrates strong verbal and written communication and organizational skills.
• CPR certification is required.
• Licensed driver with a vehicle in good working order.

Responsibilities

• Assess the psychosocial status of patients and develop a plan of care to meet patient goals.
• Complete psycho-social evaluations and plan interventions.
• Maintain timely clinical documentation on all patients referred to social work.
• Complete clinical assessments and progress notes of visits and patient responses to the plan of care.
• Provide information and referrals for patients and families regarding needs.
• Serve as liaison to patients, families, and community agencies.
• Maintain collaborative relationships with staff and IDT members.
• Develop contacts with public and private agencies as resources.
• Participate in development of total plan of care and case conferences.
• Provide education on advance directives to patients and families.
• Assist physicians and team members in understanding social factors impacting health.
• Participate in discharge planning as needed.
• Advocate for patients with internal and external personnel.
• Support Bereavement program as directed by Hospice Manager.
• Attend internal and external trainings and meetings.
