Job summary

Central Coast VNA & Hospice (VNA) in Monterey, CA, is seeking a part-time Medical Social Worker to support our Home Health and Palliative Care patients. The role evaluates social and emotional factors affecting illness, and collaborates with the care team to develop individualized plans.

Qualifications include a Bachelor of Science in Social Work with healthcare experience or a Master’s in Social Work, plus CPR certification and a California driver’s license with a reliable vehicle.

Qualifications

• At least 1 year of healthcare social work experience
• Current CPR certification

Responsibilities

• Evaluate social and emotional factors affecting a patient’s illness, care needs, ability to function and coping capacity.
• Collaborate with the healthcare team to develop individualized treatment plans.
• Participate in patient care conferences and care planning
