Job summary

Granite VNA, New Hampshire’s largest home health and hospice provider, seeks a Medical Social Worker (MSW) for our home-care teams across the Lakes Region. The role offers autonomy, Monday–Friday scheduling, and a focus on compassionate care for patients at home.

Qualifications

Schedule: Monday–Friday | 8am - 4pm

Location: Home Care - Lakes Region (Wolfeboro, Meredith, Laconia areas)

Pay: $35.00 - $44.00/hour + Mileage Reimbursement at the Federal Rate

Position Overview

Reporting to the Director, the MSW provides age-appropriate counseling, education, and support to patients and families in the home setting using standard social work practices. This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.

Requirements

1. Master's degree in social work from an accredited program (Council on Social Work Education) OR bachelor's degree in social work with at least one year in healthcare care setting.
