Job summary

Project FIND is seeking a Social Worker to serve older adults at Hargrave House in New York. The role involves detailed assessments, case management, counseling, housing assistance, and collaboration with tenants and staff to help residents live independently.

You will conduct intake, develop goal-oriented service plans, provide group work and crisis intervention, and document progress to ensure timely services for clients and families.

Qualifications

• Bachelor's degree in human services or related field required; MSW/LMSW preferred.

Responsibilities

• Develop training events to educate clients on self-care, health, and legal planning.
• Lead one or more support groups with psychoeducation on aging in place, health and wellness, and self-care.
• Create community events to promote wellness and socialization.
• Provide conflict resolution and address tenant concerns requiring intervention.
• Maintain timely documentation and progress notes.
• May supervise or train paraprofessional staff, including students.
• Perform other related tasks as requested.

Project FIND's mission is to provide low- and moderate-income and homeless older adults with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four Older Adult Centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. Job Summary:

The Social Worker will work with older adults, aged 62 years and older, at the Hargrave House location. The program provides case management services, information, referrals, supportive counseling, housing assistance, group work and crisis intervention for clients and their families. The Social Worker is responsible for conducting detailed assessments to determine client needs and eligibility for services and engaging with clients to develop goal-oriented service plans.

Responsibilities:

• Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits and entitlements; conduct home, office and community visits to address unmet needs.
• Implement constructive short and long-term interventions through counseling,education about counseling and physical health supports, long-term planning, and assistance with benefits and entitlements.
• Provide orientation, support, and services necessary to assist tenants to remain permanently housed in a safe environment.
• In collaboration with Property Management, assist with income affidavits,housing applications and recertifications and assist in collecting rentas needed.
• Develop and create training events to educate clients on topics that can help support them in their daily lives (e.g., self-care, health and wellness, legal planning).

• Lead one or more support groups, providing emotional support and psychoeducation about various issues related to aging in place, health and wellness, and self-care.
• Develop and create internal and external community building events and opportunities to facilitate activity and socialization that can help support clients' ongoing wellness and quality of life.
• Provide conflict resolution and engage in tenant concerns in matters requiring enhanced intervention.
• Provide timely documentation and maintenance of client files and progress notes.
• May provide supervision, training, and support to paraprofessional staff, including students.
• Perform other related tasks as requested or assigned.

Qualifications:

• Bachelor's degree in human services or related field required. MSW/LMSW preferred with at least 2 years relevant experience in social services; knowledge of benefits and entitlements a plus.
• Experience working with older adult populations and families preferred.

Schedule:

Monday-Friday from 9am-5pm. Please note this position requires availability and flexibility to occasionally work some weekends and holidays.
