Job summary

Greenwich House in Brooklyn, NY seeks a Social Worker to join Heights and Hills affiliate’s Case Management program. You will collaborate with staff to plan activities, conduct home visits, and support homebound seniors with comprehensive care planning.

The role requires MSW (or equivalent) experience, knowledge of aging, and the ability to navigate community services. Hybrid schedule; benefits include health coverage and PTO.

Qualifications

  • MSW preferred; however, candidates with equivalent relevant experience will be considered.
  • Knowledge of social work theory, interviewing principles, community resources and public benefits eligibility.
  • Awareness of the biological, psychological, and social aspects of aging and the impact of disabilities and illnesses on aging.
  • This position involves home visits in Brooklyn and requires the ability to utilize public transportation and to be able to climb stairs to visit clients.
  • Basic computer skills (Microsoft Office)

Responsibilities

  • Conduct in-home client assessments and follow-up visits to homebound seniors in need of services.
  • Develop and follow through on care plans.

  • May supervise and coordinate activities of volunteers and student interns.
  • Perform other duties as required

Heights and Hills, an affiliate of Greenwhich House, is a community based agency providing services to Brooklyn's older adults and their families for over 50 years, has an opening for qualified, committed MSW to work full-time in our Case Management program. Our mission, to promote successful aging in the community, is guided by the beliefs that older adults are assets to their communities; are entitled to live as independently as possible, with dignity and choice; and that professionally delivered home and community-based services for older people are essential. We serve homebound older adults and their families in 18 Brooklyn neighborhoods.

The Opportunity

The Social Worker works under the supervision of a Case Management Supervisor and plays a key role in ensuring that agency programs are carried out effectively and efficiently. The Social Worker will work closely with other Heights and Hills staff concerning program effectiveness and together with the Director of Programs, Case Management Program Director, Case Management Supervisors and other team members, plan and carry out agency activities and programs. The Social Worker will, with the approval of the Case Management Supervisor, attend community meetings, attend training sessions, participate in committees and work with professional staff of other agencies and organizations that provide services to the elderly.

What You’ll Bring

Employment Details

Location: Hybrid-81 Willoughby Street, Suite 302, Brooklyn, NY 11201 Schedule: 35 hours/week, Monday - Friday, 9:00 AM - 5:00 PM

FLSA Classification: Exempt Employment Type: Full-Time

Reports To: Case Management Supervisor

Salary Range

The expected base salary/hourly rate for this position is $66,500 - $68,000 annually/hourly. This range reflects the anticipated compensation at the time of this posting and may be adjusted based on experience, education, and organizational needs.
