Job summary

Peak Resources Alamance is seeking a full-time Social Worker to join our skilled nursing, memory care, and assisted living community in Alamance County. The role focuses on coordinating social services to enhance residents’ quality of life and well-being.

You will plan and implement resident-centered programs, ensure regulatory compliance, and collaborate with care teams to address emotional and social needs on an individual basis.

Qualifications

• Must have bachelor’s degree in Social Work or related field.
• One year of supervised social work experience in health care.
• Current social work license in the State if applicable.
• Excellent interpersonal and communication skills.
• Proficient computer skills.

Responsibilities

• Plan, organize, develop, and direct the Social Services Department to meet residents' social and emotional needs.

We are currently seeking a Full-time qualified Social Worker, Social Services, Degreed to join our team at Peak Resources Alamance.

Typical Work Schedule is:

Monday - Friday 8am - 5pm

Role Responsibilities

Plans, organizes, develops, and directs the overall operation of the facility’s Social Services Department in accordance with current Federal, State, and Local standards, guidelines and regulations, our P&P’s, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Requirements

Required Skills, Education, Licensure, and/or Certifications

• Minimum of a bachelor’s degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling and psychology and
• One (1) year of supervised social work experience in a health care setting working directly with individuals
• Current social work license in the State if applicable
• Excellent interpersonal and communication skills
• Proficient computer skills
• Must not pose a direct threat to the health or safety of other individuals in the workplace

Preferred Experience

• Minimum two (2) years’ experience in a supervisory capacity in a hospital, nursing facility, or other medical facility
• Minimum one (1) year experience in a long-term care facility
