Job summary

SanStone Health & Rehabilitation in North Carolina seeks an experienced Social Worker to join our caregiving team. You will assess patients, develop and execute care plans, and coordinate with staff and families to support timely progress.

You will document thoroughly, facilitate referrals to necessary services, and contribute to a compassionate, patient-centered environment that recognizes staff contributions and promotes growth.

Qualifications

Responsibilities

• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation.
