Job summary

SanStone Health & Rehabilitation in North Carolina is seeking an experienced Social Worker to join our patient care team. You will assess patient needs, develop and execute care plans, and coordinate with staff and providers to support residents.

The ideal candidate has strong communication and leadership skills, a compassionate approach, and a proven ability to build rapport with patients, families, and multidisciplinary teams.

Qualifications

• Previous experience in social work, with a focus on short term and long-term care patients.
• Compassionate and caring demeanor.
• Ability to build rapport with staff, patients, and their loved ones.
• Strong leadership qualities.
• Excellent written and verbal communication skills.

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Determine appropriate milestones to gauge patient progress.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation.
