Job summary

Hunterdon Care Center is seeking a CSW/LSW Social Worker to provide support, counseling, and case management for seniors in New Jersey. Part-time or per diem positions available with a negotiable rate of around $35 per hour during interviews.

Responsibilities include admissions assessments, care planning, Medicaid assistance, and discharge planning, with emphasis on resident advocacy and quality of care in a skilled nursing setting.

Qualifications

• Must hold a valid New Jersey Social Work Certification (CSW/LSW) in good standing.
• Strong organizational skills with the ability to multitask effectively.
• Passion for working with the senior population.
• Excellent writing and documentation skills.
• Experience working in a Nursing home Facility is preferred.
