Job summary

Hunterdon Care Center is seeking a CSW/LSW Social Worker to join our team. This part-time or per-diem role focuses on providing support, counseling, and case management to seniors in a skilled nursing setting.

Responsibilities include assessments, care planning, Medicaid applications, discharge planning, and advocacy for residents’ rights. Comfort with writing and documentation is essential; prior nursing home experience is preferred.

Qualifications

• Must hold a valid New Jersey Social Work Certification (CSW/LSW) in good standing.
• Strong organizational skills with the ability to multitask effectively.
• Passion for working with the senior population.
• Excellent writing and documentation skills.
• Experience working in a Nursing home Facility is preferred.

Responsibilities

• Conduct admission and ongoing assessments, including documentation
• Assisting residents with adjustment to the facility
• Completing and maintaining MDS 3.0 documentation
• Developing and participating in care planning
• Assisting with Medicaid applications and other financial matters
• Advocating for residents’ needs and rights
• Providing crisis intervention when necessary
• Coordinating and supporting discharge planning
