Qualifikationen

• Certification in Crisis Prevention Intervention or an equivalent de-escalation training within 6 months of hire.
• Knowledge of psychosocial assessment, crisis intervention, and discharge planning.

Aufgaben

• Meets face-to-face with patients and families to assess needs and coordinate care.
• Communicates with the care team to ensure safe, timely episodes of care and provides resources and education.
• Interprets patient/family needs and models caring practices for caregivers.
• Assesses psychosocial risk factors and supports coping and planning.
• Counsels patients and families on emotional, social, and financial aspects of illness.
• Mobolizes family and community resources to meet needs.
• Provides mental health assessments upon physician referral.
• Delivers interventions to facilitate complex discharge planning.
• Acts as liaison for abuse/neglect cases and protective services.
• Coordinates guardianship, foster care, adoption, and end-of-life care needs.
• Provides crisis intervention and access to psychiatric referrals or resources.
• Engages in de-escalation during dangerous or high-risk situations.
• Supports patients through acute crisis with de-escalation techniques.
