Qualifikationen

• MSW degree and active LSW licensure required.
• CPI certification or equivalent de-escalation training within 6 months of hire.
• Experience with psychosocial assessment, crisis intervention, and discharge planning.
• Knowledge of protective services liaison and guardianship/adoption processes.
• Proficiency with electronic medical records.

• Meets face-to-face with patients and families to assess needs and coordinate care.
• Collaborates with care team to ensure safe, timely care episodes and provide resources/education.
• Interprets complex needs and models compassionate care to others.
• Assesses psychosocial risk factors and coping abilities.
• Counsels patients and families on emotional/social/financial consequences of illness.
• Accesses family/community resources to meet identified needs.
• Provides mental health assessments upon physician referral.
• Facilitates complex discharge planning and crisis intervention.
• Serves as liaison with protective services for abuse/neglect cases.
• Supports guardianship, foster care, adoption, and end-of-life considerations.
• Provides crisis intervention and psychiatric referral recommendations as needed.
• De-escalates dangerous situations and leads clinical interventions.
