Acadia Healthcare in Fort Wayne, IN is seeking a licensed social worker to plan, direct, and coordinate therapeutic support for patients with behavioral health needs across the care continuum. The role focuses on comprehensive psychosocial assessments, treatment planning, crisis intervention, and family education.

You will work within a multidisciplinary team, coordinate patient placements, supervise interns, and ensure accurate charting and regulatory compliance.

Qualificações

• Master's degree in Social Work from an accredited college or university required.
• CPR and de-escalation/restraint certification required.
• State licensure (LMSW, LCSW, or LSW) required.
• First aid may be required depending on state or facility.

Responsabilidades

• Conduct psychosocial assessment of patient and develop a treatment plan.
• Obtain information from patient and family through observation and interviews.
• Provide crisis intervention and assist families in understanding implications of life situations.
• Participate in multidisciplinary treatment team meetings for planning and discharge.
• Identify and coordinate patient needs related to illness, housing, finances, education and community services.
• Facilitate education, process groups, support groups and referrals as requested.
• Supervise social work interns and provide guidance to staff.
• Maintain accurate documentation and ensure compliance with standards and accreditation.
