Job summary

Saint Elizabeth Community is seeking a Per Diem Social Worker to visit patient homes across Rhode Island, providing evaluations and information on resources to help clients stay at home safely while maintaining quality of life.

As a member of an interdisciplinary team, you will document findings in our software using an iPad, communicate professionally by phone and email, and contribute to compassionate, person-centered care.

Qualifications

• Licensed Clinical Social Worker in the state of Rhode Island.
• Demonstrates ability to document findings and outcomes clearly.
• Communicates professionally in person, by phone and email.

Responsibilities

• Visit patient homes across Rhode Island to provide evaluations and information on resources to help clients remain at home safely while maintaining quality of life.
• Document findings and outcomes via company software using an iPad.

The Per Diem Clinical Social Worker for Saint Elizabeth Home Care is a part of a supportive interdisciplinary team and they must communicate professionally in person and via telephone and email. The Per Diem Clinical Social Worker must be able to document findings and outcomes via company software and is outfitted with an iPad.

Qualifications for a Clinical Social Worker:
