Job summary

Saint Elizabeth Home Care is seeking a Per Diem Clinical Social Worker to visit patient homes across Rhode Island, conducting evaluations and connecting clients with resources to help them stay safely at home.

You will collaborate with an interdisciplinary team, document findings in our software using an iPad, and communicate effectively in person, by phone, and by email. Flexible scheduling is available to accommodate patient needs.

The Per Diem Clinical Social Worker is responsible for visiting patient homes throughout Rhode Island to provide them with evaluations and information on resources to allow them to remain at home safely while ensuring quality of life.
