You will collaborate with an interdisciplinary team, document findings in our software using an iPad, and communicate effectively in person, by phone, and by email. Flexible scheduling is available to accommodate patient needs.

Qualifications

• Licensed Clinical Social Worker in the state of Rhode Island.

Responsibilities

• Visit patient homes in Rhode Island to assess needs and provide evaluations.
• Offer information on resources to help patients remain at home safely and maintain quality of life.
• Communicate with an interdisciplinary team in person, by phone, and via email.
• Document findings and outcomes via company software using an iPad.
