Job summary

Saint Elizabeth Home Care in Rhode Island is seeking an RN Aging Life Care Specialist for part-time days to provide one-on-one in-home care and care coordination. You will develop care plans, work with physicians and community partners, and utilize mobile tools for professional communications.

The role emphasizes compassionate, patient-centered care with flexible scheduling and opportunities for growth within Saint Elizabeth Home Care.

Qualifications

• Current unencumbered RN license in Rhode Island.
• Geriatric Care Manager certification or combination of relevant experience required.
• Minimum of three years home health experience.
• CPR certification.
• Willingness and ability to drive to patient homes throughout Rhode Island.
• Reliable transportation, current clean driver's license, automobile insurance.

Responsibilities

• Develop and maintain patient care plans to assist clients in remaining safely in their homes throughout Rhode Island.
• Collaborate with physicians, community members, and staff to ensure patients have access to resources needed.
• Use an iPhone and laptop to communicate professionally internally and externally.
