Qualifications

The Skilled Registered Nurse (RN) Case Manager - Home Care contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Skilled Registered Nurse (RN) Case Manager - Home Care conducts skilled nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Skilled Registered Nurse (RN) Case Manager - Home Care for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Skilled Registered Nurse (RN) Case Manager - Home Care is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for a Skilled Registered Nurse (RN) Case Manager:

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience, home care experience preferred.
• CPR certification.
• Reliable transportation, current clean driver's license, automobile insurance.
