Job summary

The Hamptons Center for Rehabilitation & Nursing in Southampton, NY is seeking a compassionate Social Worker to support residents and families. You will collaborate with residents, families, and the interdisciplinary team to develop care plans, coordinate services, and assist with discharge planning.

Requirements include a Bachelor’s degree in Social Work and a minimum of 1 year of relevant experience. Excellent communication and knowledge of regulations are essential for success in this

Qualifications

• Bachelor’s degree in Social Work or a related human services field.
• Minimum 1 year of social work experience, preferably in healthcare or long-term care.
• Knowledge of family assessment and intervention preferred.
• Strong written, verbal, and interpersonal communication skills.
• Knowledge of governmental regulations related to social services.
• Ability to work independently and make sound decisions.

Responsibilities

• Assess residents’ social, emotional, and psychosocial needs and develop appropriate care plans.
• Maintain accurate and timely social work documentation and resident records.
• Provide support and intervention to residents and families experiencing personal or environmental challenges.
• Coordinate and participate in family meetings and interdisciplinary care planning.
• Assist with discharge planning and coordinate services with outside agencies.
• Serve as a liaison between residents, families, staff, and community resources.
• Collect pertinent social information upon admission.
• Monitor and document social services provided.
• Promote residents’ overall well-being and quality of life.
• Perform other duties as assigned.

The Hamptons Center for Rehabilitation & Nursing is seeking a compassionate and dedicated Social Worker to support the psychosocial needs of our residents and their families. The Social Worker will collaborate with residents, families, and the interdisciplinary team to develop care plans, coordinate services, and support successful transitions and discharge planning.

Responsibilities

• Assess residents’ social, emotional, and psychosocial needs and develop appropriate care plans.
