Woburn Rehabilitation and Nursing Center is seeking a compassionate, organized, and dedicated Social Worker to join our team and support residents and families in a skilled nursing and rehabilitation setting.
The Social Worker will provide psychosocial support, assist with care coordination and discharge planning, and collaborate with residents, families, and the interdisciplinary team to promote quality of life and positive outcomes.
Complete psychosocial assessments and contribute to individualized resident care plans.
Provide support and counseling to residents and families to assist with adjustment to the facility and rehabilitation process.
Lead and support discharge planning in coordination with hospitals, families, community resources, and other healthcare providers.
Collaborate with nursing, therapy, and other members of the interdisciplinary care team.
Assist with admissions, transfers, and resident and family communication.
Ensure timely and accurate documentation in accordance with facility policies and applicable state and federal guidelines.
Participate in resident care plan meetings and follow up on assigned action items.
Connect residents and families with appropriate community resources and support services.
Advocate for resident rights, dignity, and quality of life.
Bachelor of Social Work (BSW) required; MSW preferred.
Experience in a skilled nursing facility, long-term care, or rehabilitation setting strongly preferred.
Familiarity with discharge planning and community resources.
Knowledge of resident rights and applicable healthcare regulations.
Strong communication, interpersonal, and documentation skills.
Excellent organizational and time-management abilities.
Compassionate, empathetic, and able to manage multiple priorities in a fast-paced environment.
Ability to work effectively as part of an interdisciplinary team.
