Job summary

Water’s Edge Rehab & Nursing Center at Port Jefferson in Port Jefferson, NY is seeking a compassionate Social Worker to join our interdisciplinary team. You will provide emotional support, discharge planning, care coordination, and access to community resources for residents and families.

The ideal candidate has a Bachelor’s in Social Work (Master’s preferred), experience in skilled nursing or healthcare, strong communication and organizational skills, and a commitment to resident-centered care

Qualifications

• Bachelor’s Degree in Social Work required; Master’s Degree preferred.
• Experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong communication, interpersonal, and organizational skills.
• Knowledge of discharge planning and long-term care regulations preferred.
• Compassionate, dependable, and resident-focused.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Provide emotional support and counseling to residents and families.
• Assist with admissions, discharge planning, and transition of care coordination.
• Conduct psychosocial assessments and maintain accurate documentation.
• Collaborate with residents, families, physicians, nursing staff, and interdisciplinary team members.
• Assist residents and families with accessing community resources and support services.
• Advocate for resident rights and promote resident-centered care.
• Participate in care plan meetings and interdisciplinary team discussions.
• Ensure compliance with all federal, state, and facility regulations and policies.
• Address resident and family concerns in a professional and timely manner.
• Maintain confidentiality and professionalism at all times.

Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. The Social Worker will assist residents and families with emotional support, discharge planning, care coordination, and access to community resources while ensuring residents receive the highest quality of care and support.
