Job summary

Atrium Center for Rehabilitation and Nursing in Brooklyn, NY, is seeking a Social Worker to provide essential support and advocacy for residents and families. This full-time position offers Monday–Friday, 9:00 AM – 5:00 PM, with a salary of $60,000–$65,000 annually.

The role focuses on developing social histories, coordinating resources, and collaborating with an interdisciplinary team to improve residents’ quality of life.

Qualifications

• MSW preferred
• Bachelor’s degree in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or related field
• Minimum of one year of supervised social work experience in healthcare settings; SNF experience preferred

Responsibilities

• Conduct resident and family interviews to develop social histories and care plans
• Partner with families to support emotional, social, and psychological well-being
• Assist with discharge planning, financial resources, and coordination of community services
• Collaborate with nursing, administration, and medical staff to ensure holistic care

We are seeking an experienced and empathetic Social Worker to join our team in Brooklyn and provide essential support, guidance, and advocacy for residents and their families.

Key Responsibilities

• Conduct resident and family interviews to develop comprehensive social histories and individualized care plans
• Partner with families to create programs that support residents’ emotional, social, and psychological well-being
• Assist with discharge planning, financial resources, and coordination of community support services
• Collaborate with an interdisciplinary team—including nursing, administration, and medical staff—to ensure holistic care
• Advocate for residents by addressing social and emotional needs and maintaining clear, effective communication

Qualifications

• Master’s Degree in Social Work (MSW) preferred
• Bachelor’s degree required in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or a related field
• Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
• Experience in a Skilled Nursing Facility (SNF) strongly preferred
• Strong interpersonal and communication skills with the ability to build trust and rapport
