Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking a dedicated Social Worker to join our interdisciplinary team in a fast-paced rehabilitation and long-term care environment.

This role emphasizes addressing residents' psychosocial needs, facilitating care planning, coordinating admissions and discharge planning, and supporting families to ensure safe transitions and the protection of resident rights throughout the stay.

Qualifications

• Bachelor’s Degree in Social Work (BSW), Master’s Degree (MSW), or related field preferred.
• Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of discharge planning, community resources, and care coordination.
• Familiarity with PCC and MDS 3.0 is a plus.

Responsibilities

• Assess residents’ social, emotional, and psychosocial needs.
• Develop and implement individualized care plans.
• Participate in interdisciplinary care plan meetings.
• Coordinate admissions, transfers, and discharge planning.
• Assist with securing home care services, durable medical equipment (DME), and community resources.
• Provide support and guidance to residents and families throughout their stay.
• Collaborate with clinical and administrative teams to ensure continuity of care.
• Assist with financial documentation and resident education as needed.
• Advocate for resident rights and quality of life.

Social Worker Full-Time | Skilled Nursing / Rehabilitation Salary: Up to $80,000 per year

The Riverside Premier Rehabilitation and Healing Center is seeking a dedicated and compassionate Social Worker to join our interdisciplinary team. This is an excellent opportunity to work in a large, fast-paced rehabilitation and long-term care environment focused on delivering high-quality resident-centered care.

Our facility offers a dynamic clinical setting with a strong focus on short-term rehabilitation and long-term care. With a large bed capacity and a state-of-the-art rehab program, we provide services seven days a week and foster a collaborative environment that supports both professional growth and exceptional patient outcomes.

Social Worker Position Overview

The Social Worker plays a key role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring safe and effective discharge transitions.
