Job summary

Civita Care Center at Milford is seeking a Licensed Social Worker for a 32-hour schedule to support pre-admission and admissions processes in a long-term care setting.

You will listen to residents and families, provide individual and group support, and help maintain residents’ daily physical and psychosocial needs while coordinating with interdisciplinary teams and planning discharges.

Qualifications

• Bachelor’s Degree in Social Work or a related field.
• Must be familiar with PASSARs, LOCs, updating Ombudsman Discharge Portal, and timely completion of MDS assessments.
• Strong ability to manage, multi-task and communicate effectively.

Responsibilities

• Actively involved in the pre-admission and admissions processes
• Listens to and acts compassionately with residents and their family members while providing individual and group help when needed
• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Initiating appropriate interventions for residents experiencing mental and behavioral health issues
• Identifying individual social and emotional needs
• Develop and maintains individualized care plans
• Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community

The Licensed Social Worker plays an important and trusted role in the lives of our residents and their families. As the Social Worker, you will be actively involved in the pre-admission and admissions processes.
