Cross Healthcare Services in Lakeway, TX is seeking a compassionate, licensed Social Worker to join our interdisciplinary team. The role focuses on psychosocial support, discharge planning, and ensuring compliance with CMS and facility regulations for our residents.

You will collaborate with residents, families, physicians, and the care team to promote quality of life, complete psychosocial assessments, develop care plans, and advocate for residents’ rights.

Qualifications

• Current Texas Social Worker license required (LBSW, LMSW, or LCSW).
• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred.
• Knowledge of CMS regulations and long-term care standards is preferred.
• Excellent communication, organizational, and interpersonal skills.
• Ability to maintain confidentiality and work effectively in a multidisciplinary team.

Responsibilities
